Neurodiversity-Affirming ABA × Speech Therapy: Teaching With Respect, Safety & Connection

BSP 13 從「接納」到「教導」:ABA 其實可以很溫柔——談 ND-Affirming、Stimming、AAC 與 80/20 教學法

在BSP 13的 Bilingual Speech Spot,Chloe 和 Linda(SLP+BCBA;SPABA 傳播獎得主)一起拆解了一個近年很受家長、治療師關注的問題:

ABA 可以做到「神經多樣化友善(Neurodiversity-Affirming)」嗎?

答案是:可以,而且應該。

其實很多誤解並非來自科學本身,而是來自「實踐方式」的偏差。真正的 ABA 是一套 學習科學(science of learning)——研究人類如何學習、如何被教導,而不是用來壓抑孩子、要求機械式服從。

1. Neuro-affirming=尊重 + 同意(assent)+ 安全

真正的 ND-affirming 不是「任由孩子做任何事」。
它是:

  • 尊重孩子的差異
  • 理解孩子的溝通、感官與需求
  • 取得 assent(願意配合的訊號)
  • 守護安全與生活質素

治療的目的不是把孩子「變正常」,而是 讓孩子能更舒適、更有效地參與生活。


2. Stimming:不是壞習慣,而是一種信息處理方式

孩子的 stimming(如拍手、搖動、敲打物件)通常有功能:

  • 調節情緒
  • 自我安撫
  • 尋求感官輸入
  • 表達興奮或焦慮

如果沒有危險,我們可以接納。
如果 stimming 會導致受傷(如拍硬桌、撞頭),我們就引導孩子做更安全的替代:

  • 拍軟墊
  • 拍鼓
  • 拍枕頭

這不是壓抑,而是 用更安全的方式滿足同一個需要。


3. AAC:先探索,再示範,然後等待

對很多家長和初階治療師來說,AAC 可能令人有點緊張:「如果孩子亂按怎麼辦?」

但「亂按」其實是探索的一部分。

有效的 AAC 教學流程是:

示範(Model) → 等待(Wait) → 撤助(Fade)

  1. 先讓孩子探索,理解它是一個可以發聲的工具
  2. 大量示範(80% modelling)
  3. 加入動機(temptation)
  4. 延長等待,讓孩子有機會主動
  5. 慢慢撤除提示,最高目標是獨立溝通

AAC 不是「按對才算」,而是打造 主動、自由、有效的溝通權。


4. 80/20 原則:治療中最好用的黃金比例

我們在 podcast 中提到治療中非常實用的 80/20 教學法:

  • 80%:容易、興趣導向、示範
  • 20%:目標性教學、加入難度

這個比例可以:

  • 保持孩子的參與度
  • 建立成功感
  • 減少挫折
  • 創造自然的學習機會

隨着孩子的投入度增加,可以慢慢變成:

70/30 → 60/40 → 有時甚至 50/50。

治療不是「全程要求」,而是 循序漸進地教會孩子新技能。


5. 行為支援並不等於「壓抑」

ND-affirming 並不是「什麼都不做」。

當下列情況出現時,我們應該介入:

  • 孩子的 安全受影響
  • 孩子的 健康受影響
  • 孩子的 溝通權因此受限
  • 孩子的 生活質素受阻

這時的介入方式應該是:

功能等值的替代(function-matched replacement)

例子:
孩子喜歡「掟東西」→ 找出它的功能:

  • 是聲音?
  • 是視覺刺激?
  • 是手部動作?
  • 還是想吸引注意?

先理解功能,再提供 同樣功能但更安全的行為。


6. 跨專業合作:SLP × BCBA × OT 是最強組合

自閉症不是「輕、中、重」這樣的線性分類。

光譜是 一個以能力分佈為主的輪盤(wheel):

  • 溝通
  • 感官調節
  • 情緒調節
  • 社交
  • 遊戲
  • 課堂參與
  • 興趣

每個孩子的強弱項不同,因此不能用單一專業去看。

跨專業合作可以整合:

  • 行為科學
  • 語言科學
  • 感統科學

從多角度支持孩子,效果更顯著。


7. 興趣是最好的教學燃料

孩子喜歡什麼,就是最好的起點。

例如:

  • 喜歡泡泡 → 可教輪候、詞彙、身體概念
  • 喜歡車車 → 可教分類、情緒、句式
  • 喜歡琴 → 可教溝通、轉換、等待

治療不是要「把興趣拿走」,而是 用興趣開啟更多可能性。


總結:ABA 其實可以很人性化

真正的 ND-affirming 概念是:

接納差異,保護安全,以孩子為本;
同時教導他們有能力地參與世界。

**1. What Is ABA?

ABA = A Science of Learning**

  • Focuses on how humans learn
  • Challenges often come from misapplication, not the science itself
  • Goal: improving participation, communication, and quality of life

2. What Does “Neuro-Affirming” Mean?

  • Respect children’s differences and natural variation
  • Assent: the child willingly participates (verbal or non-verbal)
  • Safety first—physical, sensory, emotional, and daily wellbeing
  • Child-centred: not making a child “normal,” but helping them participate meaningfully in their world

3. Stimming: Should It Be Stopped?

If it’s safe → Accept it (hand-flapping, rocking, shaking are normal).
If there’s risk → Offer safer alternatives (e.g., hitting a drum, soft cushion, pillow).

Stimming has function:
Emotional regulation / sensory input / excitement / coping
➡️ We do not suppress needs, we meet them in a safer way.


4. How Do We Teach AAC?

Model → Wait → Fade

  1. Allow exploration
  2. Provide lots of modelling (about 80%)
  3. Use motivating activities the child enjoys
  4. Gradually extend wait-time to encourage initiations
  5. Fade prompts slowly to build independence

The goal of AAC: independent, meaningful communication.


5. The 80/20 Teaching Rule

  • 80% interest-based, easy, or modelling
  • 20% targeted teaching or new skills
    As engagement grows, shift gradually to 70/30 or 60/40.

➡️ Supports confidence, reduces frustration, and builds natural learning.


6. Clear Boundaries ≠ Harsh Control

We intervene when:

  • Safety is compromised
  • Health is affected
  • Communication access is blocked
  • Quality of life decreases

The approach:

Function-Matched Replacement

Understand the function → teach a safer, more adaptive behaviour that meets the same need.


7. Interests Are the Best Teaching Fuel

A child’s passion is the strongest starting point:

  • Bubbles → turn-taking, vocabulary, body concepts
  • Cars → categories, sentence structure, emotions
  • Music → communication, waiting, transitions

➡️ We don’t take away interests—we use them to expand the child’s world.


8. Cross-Disciplinary Support Works Best

SLP × BCBA × OT
Language × Behaviour × Sensory
Every child is a unique profile—not a straight “mild-to-severe” line.
➡️ Collaboration builds faster, more effective progress.


A Message for Parents

Acceptance doesn’t mean doing nothing.
Teaching doesn’t mean forcing.
Our goal is to help children participate safely, confidently, and in their own beautiful way.
